1. All payments and forms will be requested by our front office by phone a day prior to your scheduled appointment.
2. Upon arriving at our parking lot, STAY IN YOUR VEHICLE and dial 626.771.0064 in order to check-in for your appointment, our staff will let you know when they are ready to meet the patient at the door.
